Can't 'Check' Checkboxes in a Form?

Every time I try to click this check box, it just allows me to move it and other things I don't need to do.
I have tried turning Design Mode on and off and neither of them want to allow me to check the checkbox.
All I want to do is fill out the form but I can only edit it.

It is a .xlsm file in OO 4.1.7 on Windows 10.

Just switch OFF the edit mode of the Form control elements.

It is a .xlsm file in OO 4.1.7 on Windows 10.

And always use the native, international standard ODF file types.

You might have better success if you work with a native file (.ods) rather than a .xlsm file although in my tests it worked either way.
When you select the checkbox control, if you see the green handles surrounding it, as in your screen shot, then you are in the Design Mode.
On a spreadsheet you will need to use the Form Design toolbar which is not normally visible on spreadsheets, so you may need to add it by selecting View>Toolbars>Form Design. Near the left of the toolbar is the Design Mode On/Off button.
The checkbox should work with the Design Mode off.
