How do you create an automated template?

Does anyone know how to make an automated template such as this one, the Clickable automated resume template:
http://templates.services.openoffice.org/en/node/3073?

It prompts you field by field to fill out information to complete the document.
OpenOffice 3.1 on MacOS 10.4

  1. Insert input fields
  2. Save the document as a template.
When a document is created from a template, all fields are updated. With input fields, that means opening the input dialog. Simply opening a document will not automatically update all fields, but you can still manually request such update.
Apache OO 3.4.1/4.1.2/4.1.3, on Ms Windows 7/10 and Mac OS-X 10.8.5 and 10.11.
LibreOffice 4.3.3.2 on Mepis Antix MX 14
