Hyperlinked Table of Contents in Writer

By default, table of contents (ToC) are not hyperlinked in Writer.

Even if the document is intended to be printed only (and not exported to PDF), by default the ToC should be hyperlinked to avoid the users go through the ToC properties edition and the fiddling with the LS/LE switches (see [Solved] Make Table of Contents hyperlink?). This is a rather frequent question in the forums.
The whole string should be hyperlinked by default, up to the user to change it (to page only for example) if he wishes to.

By default, there is no reason why the entries should be colored in blue and underlined when hyperlinked.
So default style for entries should be "Index link" and not "Internet link".
